Immunization Requirements
Health Statement
Prior to registration please review the following Health Statement.
In order to maintain the health of all our pet guests, all guests must meet the following immunization requirements. Upon check-in, proof of current immunizations will be needed.
Rabies is required for every Pet on the property by State Law. They must be up to date there are no exceptions granted for dogs or cats over the age of 16 weeks.
Immunizations for Dogs:
Rabies, Parvo Virus, Distemper, and Bordetella (Canine Cough) Influenza is recommended not required. For frequent visitors such as Daycare clients 6 6-month Bordetella and fecal exams are recommended.
Dogs commonly contract kennel cough at places where large amounts of canines congregate, such as Vets Offices, Boarding and Daycare facilities, Dog Parks, Training Groups, and Dog Shows.
Immunizations for Cats:
Rabies and Distemper
Any inoculation update should be completed at least 10 days prior to boarding. Please note that we always defer to your vet’s recommendations for your pet.
If your pet is taking medications, please provide written instructions for its use.

Camelot Kennels & Pet Resort Boarding Contract
Owner agrees to pay the rate for boarding in effect on the date pet is checked into kennel as indicated on this contract.
Owner further agrees to pay all costs and charges for special services requested, and all veterinary costs for the pet during the time said pet is in the care of the kennel.
Owner further agrees that the pet shall not leave the kennel until all charges are paid to kennel by Owner.
By signing this Contract and leaving pet with kennel Owner certifies to the accuracy of all information given about said pet.
Kennel shall exercise reasonable care for the pet delivered by the owner to the kennel for boarding. It is expressly agreed by Owner and Kennel that the Kennel’s liability shall in no event exceed the lesser of the current chattel value of a pet of the same species or the sum of $200.00 per animal boarded.
The Owner further agrees to be solely responsible for any and all acts or behavior of said pet while it is in the care of the Kennel.
Owner specifically represents that he or she is the sole owner of the pet, free and clear of all liens and encumbrances.
Estimated charges for your pets stay shall be payable upon picking up pet from Camelot. Camelot shall have, and is hereby granted, a lien on the pet for any and all unpaid charges resulting from boarding pet at Camelot. The Owner hereby agrees that in the event boarding charges are not paid in when due in accordance with this contract, Camelot may exercise its lien rights upon ten days’ written notice given by Kennel to Owner by certified mail to address shown on contract. Kennel may dispose of pet for any and all unpaid charges, at private or public sale, in the sole discretion of the Kennel, and waives all statutory or legal rights to the contrary. If such sale shall not secure a price Owner specifically adequate to pay such costs of board or other charges delinquent, plus costs of sale, then Owner shall be liable to Kennel for the difference. All moneys realized by the Kennel at such sale, over and above the charges due and costs of sale, shall be paid by Kennel to Owner.
If pet becomes ill or if the state of the animal’s health otherwise requires professional attention, the Kennel, in its sole discretion, may engage the services of a veterinarian or administer medicine or give other requisite attention to the animal, and all the expenses thereof shall be paid by the Owner.
This contract contains the entire agreement between the parties. All terms and conditions of the Contract shall be binding on the heirs, administrators, personal representatives and assigns of the Owner and the Kennel.
Any controversy or claim arising out of or relating to this contract, or breach thereof, or as the result of any claim or controversy involving the alleged negligence by any party to this contract, shall be settled by arbitration in accordance with the rules of the American Arbitration Association, and judgment upon the award rendered by an arbitrator may be entered in any Court having jurisdiction thereof. The arbitrator shall, as part of his award, determine an award to the prevailing party of the costs of such arbitration and reasonable attorney’s fees of the prevailing party.
The animal is fit and healthy. Any grooming which takes place on an elderly animal or an animal with health problems will be at the owner’s risk. Grooming may expose pre-existing health and skin problems for which Camelot Kennels cannot be held liable. If pet becomes ill or if the state of the animal’s health otherwise requires professional attention, the Kennel, in its sole discretion, may engage the services of a veterinarian or administer medicine or give other requisite attention to the animal, and all the expenses thereof shall be paid by the owner.
I agree to pick up my pet at the time and date stated on the front of this contract. During the Summer months, Vacation and Holiday periods we may not be able to offer the flexibility you may need to extend your pets stay. I agree to pay an additional $100.00 per day charge for any extended boarding not noted on front of this Contract.
- Owner specifically represents to Camelot that the pet’s health care is up to date and vaccinated and has not been exposed to any communicable diseases within a thirty-day period prior to boarding. ____________ initial
- Owner agrees to allow Camelot Kennels and our representatives and employees to use his or her dog’s name and any images or likeness of his or her dog taken while the dog is staying at Camelot Kennels, in any form or format, for use, at any time, in any media, marketing, advertising, illustration, trade or promotional materials. _________ initial
- Except for Sundays and Holidays pets picked up by 10:00 AM will not be charged for that day, unless they are being groomed on a non-holiday, Mon-Sat. In this case we extend no charge pick up to 1pm. All pets picked up between the hours of 4-6pm will be charged for the day. ____________Initial
